Casey Rausin, RN, CCM
Casey prioritizes partnering with clients, families, and care teams to enhance quality of life . With a Nursing degree and board certification in Case Management, she has over a decade of experience caring for the aging population.

Outside of work, she cherishes time with her family.

Jenni Ownby, RN
Jenni is dedicated to fostering teamwork and exceeding client expectations. With experience in various nursing roles since 2008, she knows how to treat clients like family, with dignity & respect, and support her team.

Outside of work, she enjoys family time, camping, and horseback riding on their farm.
Care Coordinator
Crystal Slattery
Crystal has 4 years of experience in in case management and has previously worked in home health. She enjoys reviewing referrals & clients' needs and determining how we can help them.
Outside of work, she enjoys spending time with her four dogs who participate in various dog sports.

Dawn Groff, RN, BSN, CCM
With a BSN degree, certification in Case Management & Aging Life Care®, 15 years in home health, and 8 years as a field care manager, Dawn excels at identifying and fulfilling client needs with a personal approach to care!

Susan Robinson, LPN, CCM
Susan brings a diverse background and over 12 years of nursing experience in the Air Force. With her experience, she offers tailored solutions to her clients' needs. Her attention to detail and commitment to care exemplify our mission.

Jessica Furr, RN-BSN
Jessica got her CNA licensure in 2005 and completed a BSN in 2010. She enjoys Care Management because she can build relationships with her clients while guiding them through the aging process with dignity and grace.

Outside of work, she enjoys DIY projects, hiking, and spending time at the lake.
Care Coordinator
Sarah Walker, RN-BSN
Sarah has experience in various roles in healthcare, including urgent care, pain management & neurosurgery, long term care, and skilled nursing & rehab. She began as a certified medical assistant eventually obtaining her LPN and BSN.

Outside of work, she enjoys family time, traveling, and being outdoors.
Care Coordinator
Natalie Woodard, LMSW
Natalie has her Masters in Social Work and has been a certified mental health therapist since 2007. She has previous experience in care management roles as a home care social worker and licensed school worker.

Outside of work, she likes reading, riding motorcycles, spending time with family & friends, and volunteering.
